Nearly one week after Gia Allemand was taken off of life support, visitation and funeral dates for the reality star were announced on Sunday (August 18) by a local funeral home.

Posted on the Papavero Funeral Home site, the visitation will be held Wednesday, August 21 in Maspeth, NY with funeral services held the following day at Trinity Grace Church of Chelsea in New York City.

In addition to the service listings, Ms. Allemand's mother posted a tribute video to her late daughter, which included photos from the "Bachelor" contestants life.

On Monday, August 12, Gia was found in her New Orleans home unresponsive by her boyfriend, NBA player Ryan Anderson. She passed away on August 14 at the age of 29.

In a statement, Mr. Anderson said "Gia was the most beautiful person I knew inside and out, and she always smiled and made everyone else around her smile. She had such an amazing impact on my life and anyone that knew her was blessed."
